SEND OFF & MISCONDUCT MATCH REPORTS
Here's how it works: follow this procedure:
Don't forget: you need the Players Name, Team, & Number
You need to download the send off report from the Library.
You need to fill in the report, make sure you create an electronic signature.
You then need to email the form to the Secretary: John Hanlon to john.hanlon55@hotmail.com
You should ring John to let him know there is a report on the way.
Don't forget you can ask John or your mentor how the reports are filled in and completed in the correct manner.
